Early Detection: A Team-Based Approach

Category: Health & Wellness

Author: Dee Morse, Wellness Director

Early detection is essential to keeping residents at The Crestone Senior Living safe and well. Subtle shifts in behavior, appetite, mobility, or mood often signal that something may be wrong. When we recognize concerns early, we intervene quickly and prevent more serious complications. Early detection relies on consistent observation, clear communication, and the confidence to act. It requires a team that stays engaged and responsive.

Education That Empowers Staff

We provide ongoing education to help staff recognize what small changes truly look like. Team members may say, “That’s normal for them,” or “They’ve done this before.” However, continued training helps them understand when a behavior is no longer typical.

We emphasize how much we value the information staff share. I often remind the team that it is better to overshare and find nothing than to stay silent and miss something important. I make it clear that my office is a safe place to raise concerns. When staff feel supported, they speak up sooner.

Daily Routines That Strengthen Awareness

Stand-up meetings and rounding play a critical role in early detection. These routines help us establish a clear baseline for each resident. During stand-up, team members openly discuss observations. When one person shares a concern, others often recognize similar changes. Together, we build a more complete picture.

Rounding allows us to see concerns in real time. Because Care Partners interact with residents most often, they catch subtle shifts quickly. At the same time, my perspective during rounding sometimes helps me identify more acute changes. This shared awareness allows us to respond without delay.

Supporting Families and Preventing Disruption

Early communication with families shows our commitment to proactive care. When we address concerns promptly, we reduce the likelihood of hospitalizations and rehabilitation stays. These events can place emotional and physical strain on residents and families.

By identifying changes early, we often prevent unnecessary hospital visits. Avoiding those disruptions protects both physical and mental well-being. Most importantly, early detection helps residents remain safely in their apartments longer.

When we identify changes early and support them with timely assessment and thorough documentation, our team responds quickly, improves outcomes, and continues delivering thoughtful, person-centered care at The Crestone Senior Living.
